The first is się with the it-form of the verb: Wnioski przyjmuje się — applications are accepted. Nobody is named; it states a rule or a routine.
The second is zostać plus a form in -ny or -ty: Wniosek został rozpatrzony — the application was reviewed. It reports one event, and the form follows gender and number: została rozpatrzona, zostały rozpatrzone.
The third is być with the same form: Sklep jest zamknięty — the shop is closed. został tells what happened; jest tells the state now. Poles rarely add przez and the doer; they switch to an active sentence instead: Urzędnik rozpatrzył wniosek.
